I understand that a patch test is recommended before treatment to check for allergic reactions to pigments, numbing agents or aftercare products. If I choose not to have one, I accept the associated risk.

I understand that microblading and permanent cosmetics are forms of tattooing in which pigment is implanted into the skin using a manual blade or machine to enhance the appearance of eyebrows, lips or eyeliner. I acknowledge that the following risks and possible complications have been explained to me: infection if aftercare instructions are not followed; allergic reaction to pigments or products; uneven pigmentation, color change or fading over time; the need for touch-ups; minor bleeding, swelling, bruising or redness; possible dissatisfaction with results; and that results may vary and no guarantees have been made regarding the final outcome.

• I understand that procedures performed on or around the lips (including lip blush or lip tattooing) may trigger a recurrence of herpes simplex virus (cold sores), even if I have not had a recent outbreak.
• If I have a history of cold sores (herpes simplex), I should consult with a physician prior to the procedure.
• Antiviral medication (such as valacyclovir) may be recommended before and after the procedure to reduce the risk of an outbreak.
• The procedure may cause irritation that can activate the virus. Maintaining proper hygiene and aftercare is critical to reduce the risk of infection.
• The studio will provide aftercare instructions and may provide a lip aftercare kit to support healing; however, I accept full responsibility for following all aftercare instructions.
• I release the technician and studio from liability related to herpes outbreaks resulting from this procedure.

• Clients must be 18 years or older. Valid ID may be requested.
• Deposits (20% of the service price) are non-refundable and are applied to your total.
• 48 hours notice is required to cancel or reschedule; otherwise the deposit is forfeited.
• No-shows may be charged the full appointment amount.
• A 10-minute grace period applies for late arrivals; after that the appointment may need to be rescheduled.
• Please avoid alcohol, caffeine, aspirin, ibuprofen and blood thinners for 24 hours before your procedure (if medically appropriate).

Before-and-after photographs may be taken for documentation. Please choose whether we may also use them for training, marketing, website and social media purposes. Your name will never be published.

I understand that proper aftercare is critical to the healing process and final results. I have received and understand the aftercare instructions, and I agree to follow them: avoid sweating, swimming, saunas, makeup on the area, touching, picking and sun exposure during healing, and to inform the technician of any discomfort or issues.

I release the technician and studio from liability related to this procedure, except in cases of gross negligence. I confirm that all information I have provided is true and complete and that I have disclosed all relevant medical conditions.
